A 17 year experience of inappropriate shock therapy in patients with implantable cardioverter-defibrillators: are we getting any better?

چکیده
I mplantable cardioverter-defibrillators (ICDs) are standard treatment for both secondary and primary prevention in patients with malignant ventricular arrhythmias. 2 Inappropriate shock therapy (IST) occurs in a significant proportion of ICD patients, despite newer generation devices and algorithms specifically employed to try and eliminate this. The frequency of IST in patients implanted with cardioverter-defibrillators at this institution over a 17 year period from 1984 to 2001 was assessed. Predictive factors of IST were assessed, as was the susceptibility of certain subgroups to IST. Also, newer dual chamber devices were examined to see if they reduced IST.

A case of inappropriate implantable cardioverter defibrillator therapy induced by T-wave oversensing due to hyperkalemia.
There have been reports of hyperkalemia-induced T-wave oversensing in patients with implantable cardioverter defibrillators (ICDs). However, a comparison of T-wave amplitudes and morphologies between the surface 12-lead electrocardiogram (ECG) and ICD electrogram has not been reported. We present the case of a 70-year-old man who received inappropriate ICD shocks due to hyperkalemia-induced T-w...
متن کاملImplantable cardioverter-defibrillator shocks: epidemiology, outcomes, and therapeutic approaches.
IMPORTANCE Implantable cardioverter-defibrillators (ICDs) have revolutionized the approach to the prevention of sudden cardiac death and are commonly used in a wide range of high-risk patients, including the large population of patients with severe left ventricular systolic dysfunction. The benefit of these devices derives from their therapies, including both antitachycardia pacing and high-ene...
متن کاملImplementation of guidelines for implantable cardioverter-defibrillator therapy in clinical practice: Which patients do benefit?
PURPOSE Based on multiple large clinical trials conducted over the last decades guidelines for implantable cardioverter-defibrillator (ICD) implantations have been evolving. The increase in primary prophylactic ICD implantations challenges us to be critical towards the indications in certain patient populations. METHODS We retrospectively collected patient characteristics and rates of appropr...
متن کاملContributions of remote monitoring to the follow-up of implantable cardioverter–defibrillator leads under advisory
AIMS Automatic daily transmission of data from implantable cardioverter-defibrillators (ICDs) enables the remote monitoring of device status and leads function. We report on a 2-year experience with remote monitoring in 40 recipients of high-voltage ICD leads, prone to fracture and under advisory since October 2007. METHODS AND RESULTS The ICDs were remotely monitored as well as systematicall...
متن کاملDefibrillator implantation for the primary prevention of sudden death in patients awaiting cardiac transplantation: one center's experience.
Patients who are on a waiting list for cardiac transplantation often have a clinical profile that satisfies current recommendations for the implantation of an implantable cardioverter-defibrillator for the primary prevention of sudden death. The prospect that transplantation may take place within the short-to-medium term puts the effectiveness of this therapy in doubt. We investigated the incid...
